Patient History Forms

Due to the nature of the tests performed by this laboratory, a brief history should be included on the form and sent with the blood samples to ensure that our staff has the information necessary to perform the appropriate antibody/antigen tests for each patient.

Platelet Autoantibody Test (Direct and Indirect)

5cc Citrate (blue top)

*If the platelet count is less than 10,000 we cannot recover sufficient platelets to perform the autoantibody test (Direct). Please contact the Lab Supervisor for your testing options.
